Medical Record Request
If you would like access to your medical record, please talk to your health care team. You have a right to review, on request, a copy of your medical record and/or receive a copy of your medical record for reasonable copying charges. You may also access your medical record information quickly and electronically with MyGenesis Patient Portal account. If you do not yet have a MyGenesis Patient Portal account, you may follow the instructions on the website.
The preferred method to receive medical record request is in writing or via MyGenesis Patient Portal. However, when these options are not available, requests may be sent via e-mail or fax.
Questions regarding release of information or the pre-printed form, may be directed to the Health Information Department at 563-652-7762, Monday – Friday 7:30 am to 3:30 pm.

Ordering a Birth Certificate
Birth certificates for all individuals born at Jackson County Regional Health Center are kept on file at the Jackson County Recorder’s Office. Requests for official copies of birth certificates should be addressed to:
